Sample Job Description for a Nurse Supporting a Child with Group A Health Care Needs
Position: Registered Nurse or Licensed Practical Nurse
Duties and Responsibilities
- Wherever possible, participate in the hospital discharge planning process for the child.
- Communicate personal learning needs regarding the child’s health care to the community program administrator.
- Participate in training session(s) to address specific learning needs.
- As required, seek clinical consultation with appropriate health care professions (e.g. local Regional Health Authority, clinical supervisor of private nursing agency, etc)
- Facilitate collaboration between the child, parent/guardian agency, primary physician, relevant medical/community supports, and the community program about the child’s health needs, outcomes and interventions
- Develop and maintain child specific Individual Health Care Plan
- Collaborate with the person in the community program responsible for the coordination of activities, so that health care interventions are scheduled to minimize the child’s time away from the program
- Provide nursing intervention to support the child’s health care needs.
- Facilitate and support the child’s participation in the community program
- Participate in interdisciplinary meetings regarding the child.
- Direct parental questions regarding programming to the appropriate staff person in the community program.
- Other duties, of a non-nursing nature, as may be assigned by the community program
Qualifications
- Registered nurse with current registration with the College of Registered Nurses of Manitoba or registered licensed practice nurse with current registration with College of Licensed Practice Nurses of Manitoba (CLPNM)
- Pediatric and/or community experience
- Ability to carry out specialized tasks independently
