Services for Students Who are Blind or Visually Impaired
Consultant for BVI Services — Eligibility and Application
Eligibility
Students whose best corrected visual acuity is 20/70 or less or who have less than 20 degrees of field of vision, or whose data reveals extenuating conditions (e.g. deteriorating eye condition) as determined by the Coordinator, Blind/Visually Impaired Services Unit are eligible for services.
Application
Application for services of a Consultant for the Blind and Visually Impaired requires:
- Submission by the school of a completed Referral form with the appropriate school and parental signatures
- An Eye Report (authorized and signed by the parent/guardian) and completed by an eye care practitioner based upon an eye examination conducted within the preceding 12 months
The completed Referral and Eye Report forms are assessed by the Coordinator, Blind/Visually Impaired Services Unit and assigned to the appropriate Consultant for BVI. The assigned consultant will make contact with the school upon receiving the reports from the Manager.
